The implementation of Islamic healthcare management in regional public hospitals (RSUD) often faces significant challenges in synchronizing medical standards with the fulfillment of patients’ spiritual rights. This study aims to explore hospital facility management and environment in supporting patient worship comfort through a multidimensional perspective. The method used is descriptive qualitative with a case study approach, involving a religious leader (Ust. Toto Tohidi) and a nursing practitioner (Ibu Ida Nurhidayah, M.Kep.) as key participants through in-depth interviews. The results indicate a significant operational gap, where nurses are often hindered by the lack of standardized bedside tayamum kits and the difficulty in determining accurate Qibla direction for patients with limited mobility. The conclusion of this study emphasizes that integrating spiritual aspects into facility management is a crucial passive medical necessity to improve patients’ psychological well-being and recovery optimism. This research contributes directly to the achievement of SDGs Goal 3 (Good Health and Well-being) by realizing holistic healthcare services that focus not only on physical recovery but also on honoring individual spiritual rights. Strategic recommendations are directed towards the adoption of global quality standards such as MS 1900:2014 to create an inclusive healing environment that is responsive to the religious values of the community.
